New Jersey Launches Online Vital Records Birth Certificate Sex Designation Change Amendment App

The New Jersey Department of Health’s Office of Vital Statistics
and Registry has launched the Vital Records Birth Certificate Sex
Designation Change Amendment application, a new online service that
allows citizens to submit request forms and attestations to amend
the sex designation listed on their birth certificate to reflect
their gender identity.
The online service allows citizens to submit their amendment
request online and upload their attestation and supporting
documentation in lieu of mailing a paper application. This makes
the process easier and more convenient for both the applicant and
the Office of Vital Statistics and Registry.
The new online service can be accessed from the Office of Vital
Statistics and Registry’s website or by visiting the service’s
website directly.
“We are happy to be able to offer a convenient method for
citizens to submit their amendment requests,” said Vincent Arrisi,
State Registrar of Vital Statistics. “This new service helps make
the process of amending one’s birth certificate to reflect their
gender identity easier since the applicant can scan all of their
documentation to the application rather than mailing it in with a
paper form.”

Change Amendment service is a product of a partnership between the
state of New Jersey and the New Jersey Information Division of NIC
