Continuing Care Program will support patient recovery after hospital discharge, and ensure patients maintain their health and reduce risk of complications

CHA-Hollywood Presbyterian Medical Center (CHA-HPMC), a subsidiary of CHA Medical Group (a global leader in bio-technology and healthcare), Titanium Healthcare, LLC, and Charter Health Group have partnered together to improve quality of care as part of Medicare’s Bundled Payments for Care Improvement Advanced (BPCI Advanced) initiative. The team will launch the innovative “Continuing Care Program” at CHA-HPMC on October 1, 2018. The Continuing Care Program will provide intensive support to Medicare beneficiaries for a period of 90 days after discharge.

“It has never been more important to create patient-centric programs to optimize patient outcomes, and we are offering our new Continuing Care Program to improve the quality of care we provide for our patients — both while the patient is at CHA-HPMC, and after returning home,” said Robert Allen, CHA-HPMC president and chief executive officer. “We’re on a continual mission to create better health programs for our patients — and part of it is driven by our ability to deliver innovative solutions. Hollywood Presbyterian is making huge investments in the facility, technology, and people to better serve our patients and be the provider-of-choice in the Los Angeles community.”

The CHA-HPMC Continuing Care Program is designed to ensure that patients maintain their health and reduce the risk of complications. The Continuing Care Program will support patients with a diagnosis of sepsis, with the potential to expand the service to other diagnosis groups in the near future. Goals of the program include stringent quality measures, patient satisfaction, and avoiding hospital readmissions. The Continuing Care Team includes patient navigators, case managers, and a 24/7 nurse triage line to jointly provide well-coordinated, high-quality, and patient-centered health care. Eligible patients will be assigned to a clinical pathway associated with a customized care protocol.

This voluntary BPCI-A program is a vast improvement compared to traditional Medicare payment models because it creates incentives for providers to work together to coordinate care both during and after the hospital stay. The Continuing Care Program will significantly improve care coordination and improve the patient experience.

Central to this program are the collaborative partnerships that CHA-HPMC formed with Avant-garde Health, Collective Medical Technologies, Titanium Healthcare, and Charter Health Group — where post-acute care will be coordinated across these multidisciplinary teams using innovative processes and technology.

“Titanium’s 30-day readmissions rate, a key measure for health plans, has been less than 2% in downtown Los Angeles, where the national average is over 20%,” said Gray Miller, Titanium founder and chief executive officer. “We know that with our people, processes, and technology, we can make substantial improvements in the transition from the hospital setting to primary care. That is our key focus area.” Formed in 2015, Titanium Healthcare was started with the simple goal of creating healthcare the way a patient would want it and has delivered exceptional results ever since.

“Charter Health is thrilled to partner with CHA-Hollywood Presbyterian Medical Center and its affiliates on the BPCI-A initiative, and we look forward to coming together to improve the health and well-being of the community,” said Fred Frank, Charter chief executive officer. “We are committed to developing a meaningful and successful partnership, ensuring our patients experience a smooth transition from the hospital to home and receive continued access to our 24/7 clinical support team. Our care models are aimed at enhancing patient care, through accessibility, immediate patient engagement, and a high-touch approach. These home-based complex care programs have been highly successful at improving patient experience and optimizing resources for hospitals and health systems.”
