Continuing Care Program will support patient
recovery after hospital discharge, and ensure patients maintain
their health and reduce risk of complications
CHA-Hollywood Presbyterian Medical Center (CHA-HPMC), a
subsidiary of CHA Medical Group (a global leader in bio-technology

have partnered together to improve quality of care as part of
Medicare’s Bundled Payments for Care Improvement Advanced (BPCI
Advanced) initiative. The team will launch the innovative
“Continuing Care Program” at CHA-HPMC on October 1, 2018. The
Continuing Care Program will provide intensive support to Medicare
beneficiaries for a period of 90 days after discharge.

The CHA-HPMC Continuing Care Program is designed to ensure that
patients maintain their health and reduce the risk of
complications. The Continuing Care Program will support patients
with a diagnosis of sepsis, with the potential to expand the
service to other diagnosis groups in the near future. Goals of the
program include stringent quality measures, patient satisfaction,
and avoiding hospital readmissions. The Continuing Care Team
includes patient navigators, case managers, and a 24/7 nurse triage
line to jointly provide well-coordinated, high-quality, and
patient-centered health care. Eligible patients will be assigned to
a clinical pathway associated with a customized care protocol.
This voluntary BPCI-A program is a vast improvement compared to
traditional Medicare payment models because it creates incentives
for providers to work together to coordinate care both during and
after the hospital stay. The Continuing Care Program will
significantly improve care coordination and improve the patient
experience.
Central to this program are the collaborative partnerships that
CHA-HPMC formed with Avant-garde Health, Collective Medical
Technologies, Titanium Healthcare, and Charter Health Group — where
post-acute care will be coordinated across these multidisciplinary
teams using innovative processes and technology.
